As for what the definitive answer might be, really look at the capture and think back to what we've often discussed. For me to say any more would be giving it away...

The shock of seeing or making fun of having lit cigarettes around oxygen/oxygen tents on DS is a long running joke on the forum, Magnus - going all the way back to 2002 (on this incarnation of the forum - and probably all the way back to 1999 on the VantageNet incarnation) - and it always comes up whenever Willie's hospital scenes are discussed. Perhaps it's just that you haven't ever come across those topics.
