So far, it is a dream/blurred reality/possible threat all mixed into one. Allowing himself to attack Megan would have made it too real for Barnabas. The leviathans are trying to show Barnabas how powerful they are and what they can do to him if he misbeahaves. Becoming a vampire again is no doubt his greatest fear.
